Shipping Insurance - Customer Decides at Checkout

Hello,

It would be a nice feature to set up shipping insurance as an option -- the customer decides.

Under General Shipping Settings it is all or none. But would be nice if we let the customer choose whether they want to use additional insurance at checkout, especially on higher value carts.

  I like this idea! It would also be nice to have a minimum amount, and the ability to force insurance for orders over a certain $ amount!
