HOTFIX: Fixed Issue With Adding Phantom Items To Order Edit

Corrected an issue that would prevent adding phantom items to the cart via Order Edit when custom shipping methods were in use.
Fixed a display issue with the Order Edit phantom item popup where it was too small and the "request time" text appeared on top of the "add phantom item" button. The latter has been removed entirely from the popup.
