HOTFIX: Custom Fields Improvement

Order custom fields set to display with the address info will now save properly.
Also fixed some styling-related issues with custom fields shown in the address areas.
These fields now look like they are part of the table, not a separate one that doesn't adhere to the styling. The required * will also show if they are required.
